A Case Report of Transient Stomatocytosis and Hemolysis Associated with Alcoholism



Abstract
In 1970, Douglass and Twomey first described four cases of transient stomatocytosis
and hemoglysis associated with acute alcoholism. Presented here a case with transient
stomatocytosis and hemolysis directly related to alcohol abuse. With the best of our
knowledge, this is the first case reported in this country.
A 37 year old man was admitted to YUMC because of anorexia, nausea, and
dizziness, which were preceded by excessive alcohol drinking. Peripheral smear revealed
pancytopenia, reticulocytosis and 5.1-25% of stomatocytosis. 51Cr-labeled
red cell half life survival time was shortened (T¨ö : 12 days) and serum folate and red
cell folate decreased. Bone marrow aspirates demonstrated hypocellular marrow and
erythroid hyperplasia. Liver function testis showed mild hepatic dysfunction without
evidences of hypersplenism. Osmotic fragility test and autohemolysis test were within
normal limit. While the patient war under hospital supervision and abstained from
drinking stomatocytosis disappeared and other hematologic findings got improved.
